PATIENT UPDATES FOR THE COVID-19 OUTBREAK

• If you are experiencing dry cough, fever, or shortness of breath before your appointment, please stay home and call us at (914) 835-6604.

• At present, we are continuing with all scheduled appointments. The office is scheduling patients with less frequency to maintain proper disinfection between patients and to adhere to social distancing standards. We intend to remain open as long as we can ensure the safety of our patients and staff. We are actively monitoring the guidelines set by the CDC and New York State Public Health Department.

• If you are over age 60 or if you have conditions that put you at high-risk: If you are in need of ESSENTIAL OR URGENT foot care, please keep your appointment. Please consider rescheduling ROUTINE foot care appointments for a later date.

• We are open for routine care, as well as all urgent visits as long as you are in good health. Do not hesitate to call with any questions or if you are in pain.

• We are now scheduling tele-health appointments: If you have a smartphone with a camera and FaceTime, Skype, WhatsApp, OR a computer with webcam capabilities, you can now meet with one of our physicians from your home. Call our office to schedule your appointment.

New Patient Information

New patients can complete their intake form online before their first appointment or fill it out upon arrival. Please arrive 15 minutes prior to your first appointment to complete forms.

To complete our new patient form, please reference the email you received. If you have not received it, please call our staff.

insurances accepted

We accept most insurance and managed care plans. The plans we DO NOT accept are: Fidelis and Affinity, WellCare, Child Health Plus through Empire BCBC, Empire BCBC through the exchange, and HIP (Millennium Network).
